What is Benfleet Primary Care Network?

A Primary Care Network (PCN) is a new way of organising care, bringing together all health and care services in a local area to deliver more joined up care to a population, usually covering around 50,000 people.

Benfleet PCN covers our local population and includes General Practice (GP), pharmacies, community health services, mental health services, adult social care and voluntary organisations. The majority of PCNs were created in 2019 as part of the NHS Long Term Plan which proposed a new way of delivering health and care services.

Our Benfleet PCN team is made up of just over 30 healthcare professionals such as First Contact Pratitioners, Advanced Clinical Practitioners, Emergency Care Practitioners, Clinical Pharmacists and Mental Health Nurses alongside our administrative and management team.

You may receive a call or have an appointment booked with one our PCN members of staff who are there to support you and treat you as part of, and working together with your registered GP surgery.
