Register as a New Patient

Patients living within our practice catchment area (see map opposite) may register with the practice when the practice list is open.

Please see attached Practice Leaflet with information about both surgeries at Rushbottom Lane. 

The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Dr Khan & Partners are now using a new online service called [Register with a GP surgery] that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Alternatively you can register with either Practice, by bringing in a form of ID i.e. passport or drivers licence along with your medical card (if you have one) or complete a patient registration form and a patient questionnaire, which can be obtained at reception. 

Please put as much information on the forms as possible, this speeds up the registration process. Your notes from your previous practice will then be requested to be sent here. This can take six weeks or longer.

Please ensure that you have at least 30 days supply of any regular medication you are taking prior to leaving your current practice to avoid being without your medication. 

New patients registering with the practice will be offered a health check within the first three months of registering.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Parking and Access

A car park is provided for limited access to patients who hold a disabled blue badge or have mobility problems. 

Do not park in the hatched area or on the double yellow lines. (This needs to be clear for Emergency vehicles). Please park on Rushbottom Lane and only in the laybys outside of 08:30 - 09:30 and 14:45 - 15:45
